1: Complete Applicant Registration Packet

Our Applicant Registration Packet is required at all schools to ensure compliance of child’s safety and needs while attending school.

2:Withdrawal Form / Letter

Form or letter from previous public or private school, if applicable. Form must be dated and signed by the parent and school representative.

3: Student’s Birth Certificate

An official and legible copy for verification of student’s legal name and age.

4: Florida Certification of Immunization

The Blue DH 680 found in Code B Forms (Check Expiration Date). This can be completed by your child’s physician or the Department of Health (CLICK HERE)

5: School Entry Health Exam

The Yellow DH 3040 Physical Exam Date is required within one year of school entry.

6: Proof of Residency

The three acceptable forms of Proof of Residency are:

  • Current Homestead Exemption Card/Mailed Notification or Property Tax Statement or Signed Closing Contract

  • Rental/Lease Agreement

  • Verification Form from School District Student Enrollment.

7: Verification of Academic History

We accept a transcript, withdrawal form or last report card.

8: Verification of Special Education Information (if Applicable)
